Project Basics

Narrative Section:

1. Community description

Describe the community (county, town, neighborhood, etc.) represented. Include population, geographic size and location (if not defined by unit of government--city, county, etc.), and any relevant characteristics such as income levels and race/ethnicity that support the need for and viability of the proposal. If possible, provide specific information about the number of young children (0-6) including number of births in the community for the recent year.

2. Description of community assets

Describe current community resources and other assets which could support young children's physical activity and healthy nutrition. It is helpful to include some specifics about the organization of child care, health services, recreational opportunities, and similar systems.

3. Demonstration of community support

How is the community demonstrating interest and support for this work? (see examples in the RFP).

4. Description of collaborative

Briefly describe the collaborative. Include member organizations, history, purpose, past accomplishments, and specifically how this proposal fits the work of the collaborative. If new members are being added for this proposal, explain proposed new member organizations and what each would bring to the work. Be sure to indicate the organization that is serving as the fiscal agent and the reasons this organization has been selected to be the fiscal agent for a grant. (Involving multiple sectors--such as transportation, schools, child care, parks and recreation--is viewed positively.)

5. Key individuals

List key individuals and the background and role of each in planning and implementing the proposed project.

6. Project description

Describe the proposed project. The goal is to create a word picture of what will be done and who will be doing it (when it will be done will be addressed in the timeline below). This response should be one of the lengthier in this proposal. If this is part of a larger plan adopted either by the coalition or the community, please give a brief description of the more comprehensive plan and how this project advances the purposes of the overall plan. (If this was explained previously, there is no need to repeat the information.)

7. Scope of project

If the proposal addresses only physical activity or only nutrition and the reason was not explained in the project description, please explain why one or the other was chosen and how the one not chosen is being addressed in the community.

8. Result expectations

Given the relatively short time-frame of a grant (1-2 years), what concrete results do you expect? Please explain your "theory of change" or "logic model" to help us understand how what you propose to do will lead to the results you expect and how these results will lead to improved physical activity/nutrition for young children. Include any evidence to support the effectiveness of the proposed approach (similar program was used effectively in another community or in another area of work, based on an evaluated program, etc.). This is not a literature review but a concise description of documented, evidence-based best practices which motivated your community to adopt this approach.

How many young children will be impacted by the project?

10. Evaluation of effectiveness

What will be measured to evaluate effectiveness of the project and how will this be accomplished? This should tie directly to your response to question 8.

11. Quality assurance

If not addressed previously, how will the quality of services, materials, programs, etc. be assured? (Surveys, adopt proven materials, etc.)

12. Sustained community benefits

What benefits to the community will be sustained beyond the grant? (See explanation of sustainability in the RFP.)

13. Project timeline

Provide a brief timeline for the project

14. Tiers addressed by the project

This proposal is designed to create changes in the community in the following categories: (please mark all that apply and provide a two or three sentence explanation of how each tier below is being addressed.

15. Special budget question

If the budget for the project requests more than 50% for capital items, explain why this is reasonable for this project (note that your response to question 14--addressing multi-tiers--is critical to allowing this capital expenditure)
