Frequently Asked Questions:

  • Can I start my RE-AIM and log back in later to continue?
    • Yes, all you need to remember is your login name and password
  • How will I know my RE-AIM was successfully submitted?
    • When you've successfully submitted the RE-AIM report, you will see a confirmation page. If you enter an email address during confirmation, you will receive an automatic confirmation email from the system.
  • What is required to complete the RE-AIM report?
    • For each of your HCIA work plans, you must first view and update the plan (if no changes, just mark as done during save - see instructions) AND you must complete a RE-AIM survey for that plan.
    • Once each plan has been updated and each RE-AIM completed, follow directions to submit the full report. You will not be able to submit until all requirements are completed.
  • Any tips for using the online form?
    • Yes, be sure to save any changes you make before leaving the page you are on. The system will try to prompt you if you try to leavea page without saving changes, but the warning is intended as a backup only.
    • If you are writing long text passages, please save your work every few minutes. The system does everything possible to prevent loss of unsaved data, but cannot protect against all computer or network errors that may crop up.
