Process overview

If you submitted a Letter of Intent and received an invitation from the Health Fund, use this online form to complete and submit your invited proposal. Please feel encouraged to contact us if you have any questions or if you would like to discuss your application with our program staff at any point.

Invited proposals will be reviewed by the Health Fund's Programming & Evaluation Committee, which submits recommendations to the Board of Trustees. Final decisions are made by the Board.

Instructions for completing a grant application
Due Date: July 7, 2008

The following information provides instructions for completing an Innovation Funding grant proposal.

1) Complete this online proposal form no later than July 7.

A link with an authorization code has been emailed to you. You may want to bookmark this page for easy returns. Use the same login name and password from your Letter of Intent and you will be able to log off and on to the form as needed. Be sure to save information on the form frequently.

The basic information you provided with your Letter of Intent has been transferred to your grant application so that you do not have to fill in this information again. You may, of course, update any information on the form that has changed.

The form should be self explanatory. You are not limited by number of pages; however it is best to answer the questions directly and succinctly.

The budget should be for the entire project and all entries should be rounded to the nearest dollar. Remember that indirect expenses are limited to no more than 10% of the total. On the budget narrative/justification, please detail what is included in each expense line item and indicate how the expense item supports the achievement of the project.

To submit your completed application, simply push the “Submit Now” button. You will receive an emailed acknowledgement that your application has been received soon after you submit it. If you do not receive an acknowledgement, please call the Health Fund office to make sure the application has been received.

2) Mail supporting documents by first-class mail postmarked by July 7.

One copy of each of the following supporting documents should be postmarked and mailed (by regular first-class mail) no later than July 7 to complete your application. Items below marked with ** are not applicable to University applicants.
a. Cover Letter that includes a signature from the authorized officer of the applicant organization
b. Support letters indicating commitment of other organizations essential to the project
c. Research references supporting the project (optional)
d. Formal logic model (optional)
e. IRS Letter of Determination**
f. IRS Form 990; include all pages**
g. Most recent annual audit, including notes**
h. Current financial statements (within 2 months of application); include income statement and balance sheet**
i. List of the organization’s governing board with terms of service and occupation**

Process Timeline
-July 7, 2008 - Proposals due
-July-August - Site visits (applicants will be contacted to set up a date)
-August - Programming and Evaluation Committee review
-September - Board of Trustees decision
-September 15 - Notification of decision to applicant
-November 10 - Funding available for approved projects

How to use this system

You will then go through a series of screens requesting information about your proposed project. Each screen will have a "Save and Continue" button which will record changes made and forward you to the next item in the form.
